Job summary

Baylor St. Luke's Medical Group seeks an experienced Clinical Pharmacist for ambulatory services to optimize medication therapy and improve patient outcomes.

You will join a multidisciplinary team, delivering evidence-based care and proactive patient management within a fast-paced ambulatory setting. Responsibilities include drug therapy management, disease collaboration, patient education, and coordination of care across services.

Qualifications

• Graduation from an accredited school of pharmacy.
• Active Pharmacist License in good standing in the state(s) of practice.
• Knowledge of evidence-based clinical pharmacy practice and pharmacotherapies.
• Knowledge of Medicare, Medicaid, and commercial plans.
• Ability to provide care appropriate to all age groups with infection control, safety, and risk management in ambulatory settings.

Responsibilities

• Prescribe pharmaceutical agents under the collaborative drug therapy management agreement, including initiating, adjusting, and discontinuing therapy.
• Collaborate with other health care providers on disease management and conditions.
• Ensure patients receive safe, efficacious, and cost‑effective medication therapy.
• Address medication-related issues to support care coordination and transitions of care.
• Order and interpret drug therapy–related tests.
• Educate patients and families on safe use of medications and non-pharmacologic management.
