Job summary

UC San Diego Health Staff II Pharmacist role delivers comprehensive direct patient care across ambulatory settings, with both virtual and in-person visits focused on safe, evidence-based medication management for patients with acute or chronic disease and complex regimens.

Under Medical Center policies and Collaborative Practice Agreements, the pharmacist provides medication management, education, counseling, monitoring, and care coordination with interdisciplinary teams to improve transitions

Qualifications

• Doctor of Pharmacy degree (Pharm D) or equivalent training.
• Registered Pharmacist in the State of California.
• Meet the minimum experience qualifications: PGY-2; PGY-1 plus 1 year; or 4 years of pharmacist experience.

Responsibilities

• Provide comprehensive direct patient care across ambulatory care settings.
• Conduct virtual and/or in-person patient visits focusing on safe, effective medication therapy.
• Collaborate with interdisciplinary teams to resolve medication-related problems.
• Support standardized medication use processes and coordinate care with retail pharmacies.
• Educate patients, monitor therapy, and assist transitions of care when appropriate.
