Job summary

UC San Diego Health seeks a Pharmacist II to provide direct patient care across transitions of care at UCSD Health, collaborating with physicians, APPs, nurses, and case managers to optimize pharmacotherapy and reduce readmissions.

As a float pharmacist, you will cover cardiology, heart failure, and medicine services, serve as a preceptor to residents and students, support quality improvement projects, and participate in committees while maintaining clinical competency and flexible scheduling,

Qualifications

• PharmD or equivalent degree required.
• Active California Pharmacist license required.
• PGY1/PGY2 or ≥4 years of pharmacist experience as an alternative path.

Responsibilities

• Participating in multidisciplinary inpatient rounds and care transitions planning to cover clinical gaps.
• Performing comprehensive admission and discharge medication reconciliation.
• Identifying, resolving, and documenting medication-related problems.
• Providing discharge medication counseling and patient education using teach-back.
• Conducting post-discharge follow-up calls and clinical assessments for adherence and access issues.
• Facilitating medication access, prior authorizations, appeals, and discharge coordination.
