St. Charles Health System seeks a Clinical Pharmacist to provide comprehensive pharmaceutical care across the hospital medication process, improving patient outcomes. You will oversee drug therapy guidance, dosing considerations, and participate in patient rounds with physicians.

Oregon licensure is required; BS in Pharmacy is required, PharmD and PGY-1 residency preferred. Experience is valued, with flexibility to work at multiple worksites as staffing needs arise.

Qualifications

• Required: Bachelor of Science degree in Pharmacy.
• Preferred: PharmD and completion of ASHP PGY-1 residency.

Responsabilités

• Provides specialized pharmaceutical services to improve drug usage and therapeutic outcomes, advising physicians on drug therapy and dosing.
• Coordinates activities of pharmacy technicians in preparation, labeling and delivery of medications per prescriptions.
• Maintains liaison with medical and nursing staff and participates in patient care rounds with physicians.
• Extracts pertinent information for the patient's medical management plan and ensures compliance with applicable regulations.
• Delivers safe, confidential care and supports continuous improvement within the department.
