Qualifications

• Doctor of pharmacy degree or relevant experience at the discretion of the hiring manager.
• State of Oregon pharmacist licensure required.
• BCPS certification must be obtained within two years from date of hire.
• BLS certification to be obtained within 90 days from date of hire.
• Must have a PGY-1 residency or four years of previous clinical practice.

Responsibilities

• Interprets treatment goals and recommends medication orders/prescriptions.
• Participates in medication prescribing under Collaborative Practice Agreements.
• Provides collaborative drug therapy management, including assessments and counseling.
• Assesses educational needs of healthcare professionals regarding medication management.
