Qualifications

• Valid RN license required.

Responsibilities

• Provides patient care by performing problem-focused assessments, planning, implementing and evaluating treatments and interventions; documents care in the EMR.
• Coordinates delivery of care within the practice and across health care settings; refers patients to needed services as ordered.
• Fosters interdisciplinary collaboration to improve quality of care and patient satisfaction.
• Communicates with patients, caregivers, and health professionals about care and the nurse's role.
• Ensures access to safe, effective, timely, efficient and equitable care; mentors new staff and supports ongoing education.
• Maintains patient confidentiality and reports concerns to supervisor; adheres to infection prevention and safety policies.
