The U.S. Department of Veterans Affairs seeks a Primary Care Clinical Pharmacy Practitioner to provide advanced clinical pharmacy services in ambulatory care across VA clinics in Colorado, under an approved scope of practice.

This intermittent position offers variable hours to meet service needs and may serve Veterans at multiple facilities, with location negotiable within the health care system's catchment.

Formación

• Graduate of an ACPE-accredited College or School of Pharmacy with a Pharm.D. degree (or BS Pharmacy historically).
• Full current license to practice pharmacy in the United States.
• English language proficiency for patient care and documentation.

Responsabilidades

• Provide advanced clinical pharmacy services in ambulatory care under a scope of practice.
• Initiate/modify medication therapy and order/interpret labs as needed.
• Collaborate with the healthcare team to optimize patient outcomes.
