Qualifications

• Graduate of an accredited school of pharmacy with a R.Ph or Pharm.D degree.
• Completion of PGY-1 residency and professional certification.
• Minimum one-year non-retail clinical experience in primary/ambulatory care; government healthcare preferred.
• Must be credentialed and in good standing with the Veterans Health Administration.
• Current, unrestricted license in the state of employment or as required by VA contracts.
• English language proficiency and solid computer skills.

Responsibilities

• Evaluates and enters provider orders for appropriateness of therapy.
• Assesses drug interactions, side effects, and questions with prescribers.
• Documents medical information in the VA patient record (VISTA/CPRS).
• Maintains veteran data confidentiality and provides patient counseling.
• Maintains records for controlled substances and urgent prescriptions.
• Supports VA guidelines and clinic-wide compliance and outreach.
