Zusammenfassung

US Department of Veterans Affairs is seeking a Primary Care Clinical Pharmacy Practitioner who operates as an Advanced Practice Provider with autonomy under a defined Scope of Practice. The position serves Veterans across multiple clinics in Colorado, with location negotiable within the VA health system's catchment area.

Applicants may be referred while education/certification requirements are pending, and must meet licensure and English proficiency standards.

Qualifikationen

United States Citizenship required; non-citizens may be appointed only as VA policy allows.
Education: ACPE-accredited College/School of Pharmacy with BS Pharmacy and/or Pharm.D.; foreign grads may qualify with FPGEC & TOEFL iBT.
Licensure: Full, current, unrestricted license to practice pharmacy in US; maintenance of licensure as required.
English Language Proficiency: Pharmacists must be proficient in spoken and written English.

Aufgaben

Clinical Pharmacy Specialist functions with scope of practice; initiates/modifies/discontinues medication therapy as needed.
Designs, implements, assesses, monitors and documents therapeutic plans to optimize patient outcomes.
Acts as consultant for intensive medication therapy management and collaborates with inter-disciplinary teams.
