The U.S. Department of Veterans Affairs seeks a Clinical Pharmacist Specialist to serve as a mid-level provider within assigned health care teams, delivering medication therapy management and direct patient care. You will design and monitor drug plans, coordinate with colleagues, and document care in the EMR.

Restriction: full licensure in a U.S. state and ACPE-accredited education; foreign grads must meet FPGEC/TOEFL; Ad-hoc telework, and EDRP participation details are provided by VA.

Qualifications

• Must be a graduate of ACPE-accredited College or School of Pharmacy with Pharm.D. or BS in Pharmacy.
• Licensure: full, current, unrestricted pharmacist license in a U.S. state/territory/DC.
• Non-US degrees must meet FPGEC and TOEFL requirements.

Responsabilités

• Design, implement, and monitor therapeutic drug plans for patients.
• Provide direct patient care and medication therapy management as a mid-level provider.
• Coordinate with health care teams and communicate findings and plans.
• Perform medication history, reviews, and reconciliation; document in EMR.
• Attend meetings and support cost-effective, evidence-based pharmacotherapy.
