Responsibilities

• Provide medication therapy management and functions as a mid-level provider with advance scope.
• Initiate, modify, or discontinue medication therapy; consultant for intensive MTM.
• Perform physical assessments and order laboratory tests to assess efficacy and toxicity.
• Collaborate with physicians, other providers, and multidisciplinary teams to optimize pharmacotherapy.
