Job summary

ProMedica Conrad Jobst Tower in Toledo, OH seeks a Clinical Pharmacist to perform ambulatory care clinical pharmacist functions and cover at ProMedica Pharmacy Medication Management sites as needed.

You will manage patients' medication therapy and communicate with patients and other pharmacists, while contributing to education of pharmacy residents, students and medical students as needed.

Qualifications

• PharmD required.
• PGY-1 residency completed or 2–3 years hospital pharmacy experience.
• RPh licensed in current state; must obtain another license within 1 year.
• BLS certification.

Responsibilities

• Ambulatory care clinical pharmacist services; willing to cover at any ProMedica Pharmacy Medication Management site as needed.
• Manage referred patients' medication therapy and communicate with patients and other pharmacists.
• Educate pharmacy residents, students and medical students as needed.
