Job summary

ProMedica Conrad Jobst Tower in Toledo, OH seeks an ambulatory care Clinical Pharmacist to manage patient medications and collaborate with healthcare teams. The role covers multiple ProMedica Pharmacy Medication Management sites as needed, including education of residents and students.

You will participate in medication therapy management, patient communication, and inter-pharmacist coordination, contributing to ProMedica’s clinical excellence in a nonprofit healthcare setting.

Qualifications

• PharmD or BS Pharm with post-2006 experience.
• 1 year of pharmacy experience.
• BLS.
• Registered Pharmacist, licensed in good standing in Ohio and Michigan.

Responsibilities

• Perform ambulatory care clinical pharmacist functions.
• Manage referred patients’ medication therapy and communicate with patients and other pharmacists.
• Contribute to the education of pharmacy residents, students and medical students as needed.
• Willing to cover at any ProMedica Pharmacy Medication Management site as needed.
