Qualifications

• NYS licensed RN required.
• Bachelor's degree in Nursing or healthcare-related field required.
• Minimum 3 years of acute care RN or RN/case management experience; or 5 years with an Associate degree.
• If holding an Associate degree, must be enrolled in a BSN program with anticipated graduation within 2 years; temporary appointment until BSN is received.
• CPR-BLS certification from AHA/Red Cross required by time of appointment.

Responsibilities

• Coordinate and facilitate patient care across the continuum to achieve optimal outcomes.
• Provide comprehensive ambulatory care coordination and ensure appropriate services are accessed.
• Demonstrate working knowledge of hospital and post-discharge services available.
• Support scheduling and care transitions as part of the care coordination team.
