Qualifications

Responsibilities

• Assist with Epic in-baskets and documentation of interdisciplinary rounds.
• Coordinate referrals and follow-up appointments; ensure required documentation.
• Assist with patient screenings and discharge planning; remove barriers to safe transitions.
• Provide administrative support to Clinical Case Managers and care coordination team.
