Job summary

Meadowbrook in Bloomer, WI is seeking a Licensed Practical Nurse (LPN) to join our Nursing department. You will provide direct resident care, administer medications, and collaborate with RNs and CNAs to meet residents' physical and emotional needs in a supportive team environment.

Responsibilities

• Administer medications and treatments according to care plans.
• Provide direct resident care including vital signs and assessments.
• Document resident care accurately in medical records.
• Collaborate with RNs, CNAs, and interdisciplinary team.
• Monitor resident conditions and report changes promptly.
• Support residents’ emotional well-being and comfort.
• Follow infection control and safety procedures.
• Attend trainings and complete duties as assigned.

• Provide direct resident care, including vital signs, wound care, and daily assessments
• Document resident care accurately in medical records
• Collaborate with RNs, CNAs, and interdisciplinary team members on care plans
• Monitor resident conditions and report changes promptly
• Support residents’ emotional well-being and comfort
• Follow infection control and safety procedures consistently
• Attend in-service trainings and professional development opportunities
• Complete additional duties as assigned by the Nursing Director or Administrator
