Job summary

Meadowbrook is seeking a Licensed Practical Nurse (LPN) to provide direct resident care, administer medications, and collaborate with the healthcare team in a supportive, team‑oriented environment.

The role supports residents’ physical and emotional needs, follows care plans, and adheres to infection control and safety procedures. Shifts include Part-Time PM 2pm-10:30pm and Full-Time NOC 6pm-6:30am, with paid time off and a 401(k) plan.

Responsibilities

• Administer medications and treatments according to care plans and facility policies
• Provide direct resident care, including vital signs, wound care, and daily assessments
• Document resident care accurately in medical records
• Collaborate with RNs, CNAs, and interdisciplinary team members on care plans
• Monitor resident conditions and report changes promptly
• Support residents’ emotional well‑being and comfort
• Follow infection control and safety procedures consistently
• Attend in-service trainings and professional development opportunities
• Complete additional duties as assigned by the Nursing Director or Administrator
