Meadowbrook is hiring Licensed Practical Nurse (LPN) to provide skilled nursing and clinical support for residents. You will administer medications, perform direct resident care, monitor conditions, and collaborate with RNs, CNAs, and the care team to ensure high-quality outcomes.

Responsibilities

• Administer medications and treatments according to care plans and facility policies
• Provide direct resident care, including vital signs, wound care, and daily assessments
• Document resident care accurately in medical records
• Collaborate with RNs, CNAs, and interdisciplinary team members on care plans
• Monitor resident conditions and report changes promptly
• Support residents’ emotional well-being and comfort
• Follow infection control and safety procedures consistently
• Attend in-service trainings and professional development opportunities
• Complete additional duties as assigned by the Nursing Director or Administrator
