Job summary

Sweetwater Care in California is seeking an LVN to provide nursing care within the scope of practice, in accordance with physician orders, center policies, and state and local guidelines. The role includes patient education, timely medication administration, and participating in rounds with physicians as needed.

The ideal candidate is a California-licensed LVN with long-term care experience; nursing management experience is preferred but not required.

Qualifications

• California LVN license is required.
• One-year nursing management experience preferred, not required.
• Long-term care experience preferred.
• PointClickCare charting experience.

Responsibilities

• Provides nursing care within scope of practice as per standards.
• Demonstrates knowledge and use of Infection Control Manual.
• Participates in Quality Improvement Program implementation.
• Uses Quality Life Manual for patient/program placement and monitoring.
• Notifies departments of patient changes (room, LOA, admits, discharge).
• Conducts family and patient education.
• Administers medications timely and per facility policy.
• Administers treatments timely and per facility policy.
• Receives, transcribes, and executes physician orders as allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state of California
• One-year prior nursing management experience preferred, not required.
