Job summary

Sweetwater Care in Healdsburg, CA is seeking a licensed LVN to provide nursing care within scope of practice, ensuring patient needs are met in alignment with standards, orders, and center policies.

You will participate in quality improvement, monitor patients, and administer medications and treatments per facility policy. Experience in long-term care and PointClickCare charting is preferred.

Qualifications

• Must be currently licensed as LVN in California.
• Long-term care experience preferred.

Responsibilities

• Utilizes the Quality Life Manual for patient identification and monitoring.
• Notifies departments of patient changes (admissions, discharges, room changes).
• Conducts family and patient education.
• Administers medications and treatments per facility policy.
• Receives, transcribes, and carries out physician orders when allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state of California
• One-year prior nursing management experience preferred, not required.
