Job summary

Sweetwater Care is seeking an LVN with California licensure to provide nursing care within scope of practice in a long-term care setting. The role includes medication administration, patient education, and collaboration with the care team.

Prior nursing management experience is preferred but not required. Experience with Point Click Care charting is desirable to document care and coordinate with physicians and other staff.

Qualifications

• Currently licensed LVN in California.
• Long term care experience.
• Point Click Care charting experience.

Responsibilities

• Provides nursing care within scope of practice according to standards, orders, and policies.
• Notifies departments of patient changes.
• Educates patients and families.
• Administers medications timely and per policy.
• Administers treatments timely and per policy.
• Transcribes and executes physician orders as allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state of California
• One-year prior nursing management experience preferred, not required.
