Job summary

Salinas Valley Post Acute is seeking a licensed Physical Therapist for a 13-week assignment in Salinas, California. The shift runs 4:00 PM–4:00 AM, three days per week, with potential extension. You will evaluate and treat SNF patients, and develop individualized plans to improve mobility and independence, while collaborating with the care team.

CA license is required; duties include Net Health documentation, and occasional float to a nearby sister facility. Competitive pay and benefits offered.

Qualifications

• Active CA Physical Therapist license is required.
• SNF experience preferred.
• Ability to work 13-week assignments with potential extension.

Responsibilities for a PT:

• Evaluate and treat skilled nursing facility (SNF) patients
• Develop and implement individualized treatment plans to improve mobility, strength, and overall functional independence
• Collaborate with the interdisciplinary care team
• Complete documentation in Net Health per facility standards
• Maintain an active CA Physical Therapist license
• Demonstrate strong documentation and time-management skills
• Maintain a flexible and collaborative mindset
• Adhere to preferred Sunday - Thursday or Tuesday - Saturday schedule
• Float to sister facility within walking distance as needed
