Your Health Organization in Albany, GA is seeking a Clinical Coordinator to support clinical needs across facilities. The role involves chart preparation, scheduling visits, logging care management activities, and assisting providers and patients.

This traveling position requires reliable transportation and daily travel between ALF, SNF, and IL facilities. Full-time, 8-hour days, 8:00 AM–5:00 PM, Monday–Friday.

Qualifications

• Must be a CMA or licensed nurse in the state of employment.
• Minimum 1 year experience in a healthcare setting.
• CPR certification required.
• High school diploma or equivalent.
• Excellent interpersonal and communication skills.
• Able to travel between offices and facilities as needed.

Responsibilities

• Review and organize patient medical records in preparation for visits.
• Ensure all necessary documentation, test results, diagnoses, and information are accessible for the provider.
• Use Navina to ensure these are added, completed, and/or updated.
• Reconcile medications.
• Collaborate with care team to schedule and coordinate patient appointments.
• Interact with patients via email, portal, phone, text, in-person to gather information and provide pre-appointment instructions.
• Utilize communication methods such as phone, email, AthenaText, Microsoft Teams.
• Document and log care management activities.
