Job summary

Mosaic Life Care seeks a Physical Therapist to evaluate and treat patients in outpatient and inpatient settings. The role involves planning treatment, educating patients and families, and ensuring documentation complies with medical guidelines.

It requires collaboration with leadership and participation in interdisciplinary teams. The PT will supervise PT assistants and students, attend staff meetings and family conferences, and contribute to process improvements while traveling between system

Qualifications

• Bachelor's Degree from an accredited Physical Therapy program is required.
• Missouri Physical Therapist license required (within 60 days).
• Basic Life Support (BLS) certification required within 90 days.

Responsibilities

• Evaluate patients referred by providers and establish goals for recovery.
• Plan and implement therapy programs using approved procedures.
• Document treatment and patient responses regularly.
• Supervise PT assistants, students, and rehab aides as appropriate.
• Participate in staff meetings, family conferences and interdisciplinary teams.
• Contribute to continual process improvement and perform other duties as assigned.

Responsibilities

• Evaluates patients referred by providers.
• Establishes goals that maximize the patient’s recovery.
• Plans a treatment program, using appropriate treatment procedures that will best accomplish the established goals.
• Implements the treatment program using approved procedures.
• Documents treatment procedures and the patient’s response to them on a regular basis.
• Responsible for treatment procedures, documentation, and supervision of physical therapy assistants, physical therapy students, and rehab aides, as appropriate.
• Participates in staff meetings, family conferences and interdisciplinary teams meetings, as appropriate.
• Participates in continual process improvement.
• Other duties as assigned

Education

• Bachelor's Degree - Graduate of accredited school of Physical Therapy - Required

Licenses and Certifications

• Physical Therapist (PT) - Licensed in State of Missouri - Required within 60 Days
• Basic Life Support (BLS) - Required within 90 Days

Travel Requirements

• Must be able to travel between various system facilities and off-site locations as needed. - Required
