Job summary

Mosaic Life Care is seeking a Physical Therapist to evaluate, plan, and implement outpatient/inpatient rehabilitation services under leadership direction. You will educate patients and families and document progress per guidelines.

The role requires Missouri PT licensure within 60 days and BLS within 90 days, with travel across system facilities as needed. A Bachelor’s degree in Physical Therapy from an accredited program is required.

Qualifications

• Bachelor's Degree from an accredited Physical Therapy program (Required).
• Missouri license as a Physical Therapist (Required within 60 days).
• BLS certification (Required within 90 days).

Responsibilities

• Evaluate patients referred by providers and establish goals for recovery.
• Plan and implement physical therapy treatment programs.
• Document treatments and patient responses regularly.
• Supervise PT assistants and rehab staff as appropriate.
• Attend staff meetings, family conferences and interdisciplinary team meetings.
• Contribute to continual process improvement.
• Perform other duties as assigned.

Job Description

The Physical Therapist (Outpatient\Inpatient) works under the supervision of leadership. Evaluates, plans and implements treatment programs for the rehabilitation of patients, as directed by a provider, using approved treatment procedures. Provides patient/family education. Participates in family conferences. Responsible for all necessary documentation as required by accepted medical guidelines.

Responsibilities

• Evaluates patients referred by providers.
• Establishes goals that maximize the patient’s recovery.
• Plans a treatment program, using appropriate treatment procedures that will best accomplish the established goals.
• Implements the treatment program using approved procedures.
• Documents treatment procedures and the patient’s response to them on a regular basis.
• Responsible for treatment procedures, documentation, and supervision of physical therapy assistants, physical therapy students, and rehab aides, as appropriate.
• Participates in staff meetings, family conferences and interdisciplinary teams meetings, as appropriate.
• Participates in continual process improvement.
• Other duties as assigned

Education

• Bachelor's Degree - Graduate of accredited school of Physical Therapy - Required

Licenses and Certifications

• Physical Therapist (PT) - Licensed in State of Missouri - Required within 60 Days
• Basic Life Support (BLS) - Required within 90 Days

Travel Requirements

• Must be able to travel between various system facilities and off-site locations as needed. - Required
