Job summary

Tanana Valley Clinic, part of Foundation Health Partners, seeks an experienced RN in Fairbanks, Alaska. The role focuses on comprehensive patient assessment, planning, and direct care within a skilled multi-disciplinary team.

Responsibilities include discharge planning, evidence-based care delivery, and ongoing patient education. Alaska RN licensure, BLS, and appropriate certifications are required; relocation and housing support are available along with health benefits.

Qualifications

• Must possess a valid current or temporary Alaska RN license.
• Knowledge and skills normally demonstrated by an Associate's degree in nursing.
• BLS certification is required.
• ACLS and/or PALS may be required depending on area of practice.

Responsibilities

• Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs.
• Formulates a plan of care including discharge planning with patient/family and health team input.
• Implements the plan of care through direct patient care and coordination of the health care team.
• Evaluates patient and family response to care, revises the plan as needed and documents in the patient record.
• Promotes continuity of care by communicating patient status to caregivers.
