Qualifications

Responsibilities

• Assesses patient needs and plans discharge in collaboration with the care team.
• Develops and adjusts the care plan with input from the multidisciplinary team.
• Provides direct nursing care per physician orders and plans.
• Communicates patient status to the team to ensure continuity of care.
• Documents assessments, plans, interventions and outcomes accurately.
• Participates in professional development and mentoring.
