Job summary

Qualifications

• Must possess a current, valid RN license in state of practice or compact licensure.
• BLS certification is required.
• Experience in the clinical area for which you are applying is desired; must have acute care experience in the past 12 months.
• Bachelor’s degree preferred; professional certification may be preferred.

Responsibilities

• Assesses patient needs across physical, psychological, social, spiritual, educational, developmental and discharge planning.
• Formulates and initiates a discharge-focused plan of care with measurable goals.
• Implements care with coordination and supervision of the health care team.
• Evaluates patient progress and revises the plan as needed in collaboration with the team.
• Documents care and communications in the patient record.
• Promotes evidence-based practice and participates in professional development.
