Job summary

Senior Lifestyle is seeking a Resident Care Coordinator to lead the Assisted Living team, schedule staff, and participate in care plan meetings with families. You will manage ISP changes and promote company policies to ensure high-quality resident care.

The role requires CNA experience, scheduling skills, and strong English communication. A GA Medication Aide certification is noted; daily operations and care coordination are central to success in this position.

Qualifications

• High school diploma or GED required.
• 3–5+ years caregiver experience preferred.
• CNA experience and scheduling experience; must communicate with families.
• Must be a State of GA Certified Medication Aide.
• Fluent English to perform duties and communicate with supervisors, coworkers, residents and third parties.

Responsibilities

• Support HWD by managing Assisted Living team members.
• Schedule Assisted Living team members.
• Participate in Care Plan meetings with families and residents and manage ISP’s as resident needs change.
• Promote all company policies by ensuring SLC communities are providing the best care possible for our residents.
• Participate in weekly at risk meetings with HWD, ED, MCC/ MCD to ensure leadership team is aware of all resident needs.
• Review and implement all necessary processes for optimal operational efficiency such as- shower schedules, laundry schedules, caregiver assignment sheets, etc.

Area of Study: General

Years of Experience: 3-5+ years caregiver experience preferred

Type of Experience: CNA experience required, scheduling, communicating with families

Special Certifications: Must be a State of GA Certified Medication Aide.

Language Skills: Acceptable fluency in English in order to perform job duties and speak, read, write, and communicate with supervisors, coworkers, residents, and applicable third parties.

Technical Competencies: Basic computer skills, Excel for scheduling

• Ability to communicate with families, frontline team, and community management team to ensure optimal operational success.
