Job summary

Circadia Health is seeking a licensed Nurse Practitioner to provide on‑site patient care across SNFs and home care networks. You will conduct initial and follow‑up assessments, train staff on Circadia workflows, and interpret AI‑driven insights to trigger timely interventions.

Role requires CA NP licensure, 2+ years of relevant experience, independence, and fluency in English. Some local travel within a 30‑40 mile radius is expected.

Qualifications

• Must hold CA NP license; ANCC or equivalent.
• 2+ years NP experience with complex adult populations; ability to practice independently.
• English required; strong communication with care teams.

Responsibilities

• Direct patient care with initial and follow-up assessments.
• Train staff on Circadia integration into clinical workflows.
• Interpret trend-based insights and prioritize high-risk patients for timely interventions.
• Complete documentation within the same day.
• Manage high-volume visits across designated sites; 25+ per day where appropriate.
• Provide feedback to improve workflows and tech integration.
