Job summary

Circadia Health is seeking a Certified Nurse Practitioner to deliver direct patient care across skilled nursing facilities and home health programs. You will interpret predictive health signals, initiate timely interventions, and mentor facility teams to integrate Circadia into clinical workflows.

The role requires CA NP licensure, 2+ years of experience with complex adult populations, and ability to travel locally. English is required; Spanish is a plus.

Qualifications

• Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid CA driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required; Spanish preferred.

Responsibilities

• Conduct initial and follow-up assessments for SNF and Home Health patients.
• Interpret trends and predictive signals to guide interventions.
• Train and coach facility staff to integrate Circadia into workflows.
• Complete documentation within 24 hours of visits.
• Manage high-volume rounds (approx. 25+ visits/day in SNFs).
• Collaborate with clinical ops and product teams for workflow improvements.
• Travel within a 30–40 mile radius; mileage and lodging covered per policy.
