Job summary

Circadia Health is hiring a Nurse Practitioner to lead proactive patient care across SNF and home health AI care programs. You will perform initial and follow-up assessments, coordinate plans of care, and train facility staff on Circadia-aided workflows.

Role requires NJ NP licensure, independence, and willingness to travel locally. You will work with clinical ops/product teams to refine data-driven interventions, aiming to reduce readmissions while delivering high-quality care in diverse

Qualifications

• Certified and licensed NP in NJ with ANCC or equivalent.
• Minimum 2+ years NP experience with complex adults.
• Ability to practice independently and travel locally.

Responsibilities

• Direct patient care with initial and follow-up assessments and care plans.
• Train staff to integrate Circadia into clinical workflows.
• Interpret trend insights and prioritize high-risk patients.
• Complete documentation within the same day.
• Round across SNFs and home visits, 25+ visits per day where appropriate.
• Provide feedback to improve workflows and integration with clinical teams.
• Support NP onboarding and collaborate with ops/product teams.
• Cover SNF sites and patient homes within a 30–40 mile radius.
