Job summary

Circadia Health, a healthcare AI company, is seeking a Nurse Practitioner to work across skilled nursing facilities and home health programs. You will see patients first, respond to predictive signals in real time, and coach facility teams to intervene earlier to prevent readmissions.

You’ll blend direct patient care with tech-to-bedside integration, training staff, and using AI insights to drive timely care. Must hold ME & NH NP licensure and enjoy a dynamic startup setting.

Qualifications

• Certified and licensed NP in ME & NH; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid ME & NH driver’s license; comfortable traveling locally.

Responsibilities

• Direct patient care: conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech - bedside integration: train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
• Close the loop: complete documentation/charts within the same day.
• High-volume rounding: 25+ visits per day across SNFs when appropriate.
• Mentor & collaborate: support NP onboarding and share best practices with clinical ops/product teams.
• Regional flexibility: cover SNF sites and patient homes within a 30-40 mile radius; mileage and lodging covered per policy.
