Job summary

Circadia Health is hiring a Nurse Practitioner to operate across SNF and Home Health AI care programs. You will see patients, respond to AI alerts, and coach facility teams to translate data into earlier interventions that reduce readmissions.

In SNFs, you’ll round on new admits and flagged patients; in home health, extend high-quality care into patients’ homes. You’ll blend direct care with tech-to-bedside integration and hands-on training.

Qualifications

• Certified and licensed Nurse Practitioner in NY (ANCC or equivalent).
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid NY driver's license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required.

Responsibilities

• Direct patient care: Conduct initial and follow-up assessments; determine plans of care; coordinate with facility providers.
• Tech bedside integration: Train and coach staff; integrate Circadia into workflows.
• Data into action: Interpret trend-based insights; prioritize high-risk patients; drive timely interventions.
• Close the loop: Complete documentation within the same day.
• High-volume rounding: Cover SNFs and patient homes within radius; around 25+ visits/day.
• Quality & product feedback: Surface field insights to improve workflows and tech integration.
• Mentor & collaborate: Support NP onboarding and partner with clinical ops/product teams.
• Regional flexibility: Cover SNF sites and patient homes within 30–40 mile radius; mileage lodging covered.
