Located in Rutland, VT with regional travel within 30-40 miles, the role combines bedside care with tech-enabled workflows and documentation within AI-powered platforms.
Qualifications
- Licensed Nurse Practitioner in VT (ANCC or equivalent).
- 2+ years NP experience with complex adult populations; able to practice independently.
- Eligible for Medicare participation; company bills on your behalf.
- Valid VT driver’s license; comfortable traveling locally.
- Thrives in a dynamic, evolving startup environment.
- English required.
Responsibilities
- Direct patient care in SNF and Home Health AI Care programs; assess, plan, and adjust care when needed.
- Train and coach facility staff to integrate Circadia into clinical workflows.
- Interpret trend-based insights and prioritize high-risk patients for timely interventions.
- Document notes and charts within 24 hours of each encounter.
- Round on new admits and facility-flagged patients; manage Circadia alerts.
