Job summary

Sunnyside Community Services in New York seeks a Community Outreach Worker to promote the Case Management program among local organizations and health providers. You will build relationships, refer eligible adults to services, and coordinate with partners to expand program reach.

The role requires strong communication, bilingual abilities, and experience engaging with aging populations. A hybrid work model is offered with three in-office days and two remote days.

Qualifications

• Associate’s degree in marketing or equivalent outreach experience.
• Knowledge of social services and aging population needs.
• Bilingual abilities (Spanish, Greek, Bengali, Cantonese or Mandarin).
• Excellent written and verbal communication and engagement skills.

Responsibilities

• Collaborate with the Manager of Communications & Public Outreach to promote the Case Management program and its services.
• Engage with the community to identify and refer eligible individuals to the Case Management Program.
• Deliver presentations to agencies, practitioners, and community groups in-person, via zoom or social media.
• Serve as the main referral source for the Case Management Program.
• Be available for occasional evening or weekend outreach events, presentations, or community engagements.
• Perform other duties as assigned.
• Document outreach activities and referrals for program evaluation and reporting purposes.
