Job summary

Sunnyside Community Services, Inc. seeks a bilingual Community Outreach Worker (full-time, temporary) to raise awareness of Case Management services among local organizations and community groups in New York, connecting older adults to needed services.

The role requires strong communication and relationship-building skills, with a hybrid schedule (three days in office, two remote), occasional evening/weekend events, and duties including referrals, outreach tracking, and presentations.

Qualifications

• Associate degree in marketing or equivalent; HS diploma with 2+ years outreach.
• Knowledge of social services.
• Experience working with aging population.
• Familiar with communities Districts 1–5.
• Excellent written and verbal communication skills.

Responsibilities

Community Outreach Worker (Bilingual; Full-Time; Temporary)

The Community Outreach Worker will build awareness of SCS' Case Management services among local organizations, healthcare providers, social service agencies, and community groups. The right candidate will have strong communication skills, experience with relationship building, and organizational skills to effectively connect older adults with services and coordinate with community partners to promote program services.

SCS currently operates on a hybrid work model with three days in the office and two days remote.

Responsibilities

• Collaborate with the Manager of Communications & Public Outreachto promote the Case Management program and its services.
• Engage with the community to identify and refer eligible individuals to the Case Management Program.
• Deliver presentations to agencies, practitioners, and community groups in-person, via zoom or social media.
• Serve as the main referral source for the Case Management Program.
• Be available for occasional evening or weekend outreach events, presentations, or community engagements.
• Perform other duties as assigned.
• Document outreach activities and referrals for program evaluation and reporting purposes.

Qualifications

• An associate's degree in marketing from an accredited college/institution; a high school diploma with two or more years performing outreach activities.
• Knowledge of social services.
• Experience working with the aging population.
• Knowledge of communities within Districts 1,2,3, and 5.
• Excellent communication skills (written and verbal).

Physical Requirements and Working Environment

• Travel by all forms of public transportation or car in the community; ability to use stairs if there is no elevator available.
• Ability to communicate orally and in writing with outside agencies, clients, staff, co-workers, and vendors.
• Good organizational skills.
• Ability to track and maintain accurate data.

Classification: Non-Exempt Full Time/Temporary until June 30, 2027

Reports to: Case Management Director

Salary: $48,000 - $50,000 annually
