Job summary

National Church Residences is seeking a Service Coordinator to support residents at Ashley House Apartments in Valdosta, GA. You will coordinate programs and services to help seniors age in place, partnering with community providers and documenting services.

The role emphasizes person-centered care, timely assessments, outreach, and building informal support networks, while maintaining compliance with HUD and accrediting standards.

Qualifications

• Requires high school diploma or GED; bachelor’s degree preferred.
• Two or more years of experience in social service with elderly or disabled.
• Good communication, comprehension and interpersonal skills.
• Working knowledge of Windows, Microsoft Office and internet tools.

Responsibilities

• Coordinate programs and activities for eligible individuals.
• Liaise with community agencies, network with service providers and negotiate discounts.
• Document participant services in the management system and manage supportive services.
• Develop and maintain a Resource Directory of local service providers.
• Organize educational events and promote participation for participants.
• Identify partners to provide health services and screenings at participants’ homes.
• Assist participants in building informal support networks and coordinate volunteer involvement.
• Conduct assessments and referrals while following program requirements and HUD/accrediting standards.
