Job summary

National Church Residences is seeking a Service Coordinator I in White Rock, NM. This on-site role coordinates services for eligible participants, liaises with community agencies, and conducts person-centered assessments to help residents age in place.

You will document services, develop care plans, and collaborate with the Property Manager and Program Manager to ensure quality of life for participants.

Qualifications

• High School Diploma or GED required; bachelor’s degree preferred.
• Two+ years of social services experience with elderly or disabled.
• Good communication and interpersonal skills; proficient with Windows and MS Office.

Responsibilities

• Coordinate programs and activities for eligible individuals.
• Liaise with community agencies and identify service providers.
• Conduct participant assessments and document findings per program requirements.
• Develop and maintain care plans, monitor referrals and services.
