Job summary

Colorado Health Network is hiring a Health Access Aging Services Coordinator in Denver to support older adults living with HIV by helping with health insurance access and related services. The role manages 100+ clients and collaborates with CHN departments, partners, and state agencies to ensure smooth transitions and warm handoffs.

The ideal candidate is organized, compassionate, and knowledgeable about aging-related issues, HIV/AIDS, and public health systems, with strong communication skills

Qualifications

• Knowledge of HIV/AIDS, health insurance systems, and aging issues.
• Experience assisting with healthcare navigation and benefit programs.
• Bachelor's degree in social work or related field.
• Excellent organizational and time management skills.
• Strong interpersonal and communication skills.
• Bilingual or multilingual, especially Spanish, encouraged.

Responsibilities

• Educate clients on SDAP and updates to healthcare policy.
• Conduct enrollment appointments for clients aged 50+, including Connect for Health Colorado, Medicaid, Medicare.
• Provide quarterly outreach and ongoing Medicare assistance.
• Develop individualized Medicare and SSDI transition plans.
• Help clients complete necessary enrollment paperwork.
• Coordinate invoice processing and bill payments for eligible clients.
• Work with pharmacy and clinic partners to resolve access issues.
• Maintain accurate data in electronic systems.
• Serve as healthcare access and insurance navigation resource.
• Foster positive relationships with clients and partners.
