Job summary

Area Agency on Aging & Disabilities of Southwest Washington (AAADSW) seeks a dedicated ADRC professional to serve as the initial contact for services for older adults, people with disabilities, and family caregivers. The role emphasizes phone and computer-based information, eligibility screening, and connecting clients with resources to support independence at home and in the community.

You will conduct outreach, complete applications for benefits, and collaborate with partners to coordinate

Qualifications

  • Four years of paid, full-time experience in social, human, or customer service roles providing information and navigation assistance to vulnerable populations, with a high school diploma or equivalent; OR
  • Three years of paid, full-time experience in such roles with bilingual/bicultural experience; OR
  • Two years of paid, full-time experience with an Associate’s degree; OR
  • Two years of paid, full-time social service experience with a Bachelor’s degree; OR
  • One year of paid, full-time social service experience with a Bachelor’s degree in social services, behavioral or health sciences; OR
  • Social service experience and a Master’s degree; AIRS credentialing within two years;
  • Possession of a valid driver’s license, state-required vehicle insurance, and reliable vehicle for travel.

Responsibilities

  • Provide information by phone, email, mail, and in-person to the general public, healthcare providers, and organizations on programs and resources for older adults, disabilities, and caregivers.
  • Conduct eligibility screening for multiple programs and follow up on referrals as needed.
  • Provide person-centered options counseling resulting in successful linkage to services.
  • Assist individuals to complete applications for services and benefits (Medicaid LTSS, SNAP, Medicare Part D).
  • Promote access to programs for those with the greatest need.
  • Utilize databases to document information, eligibility, and authorization.
  • Record information in agency files and produce data reports.
  • Coordinate with community partners to ensure care alignment for clients.
  • Conduct outreach and presentations to promote programs and services.
