Job summary

AAADSW is seeking an Aging & Disability Resource Specialist to serve as the initial contact for services for older adults, people with disabilities, and family caregivers. You will handle phone and computer-based inquiries, provide accurate information about supports, perform eligibility screenings, and help connect clients to services that promote independence at home and in the community.

Qualifications

  • Four years of paid, full-time experience in social, human, or customer service roles involving information and navigation assistance to vulnerable populations, with a high school diploma.
  • Three years of paid, full-time experience with bilingual or bicultural background and a high school diploma.
  • Two years of paid, full-time experience with Associate's degree or equivalent.
  • Two years of paid, full-time social service experience and Bachelor's degree.
  • One year of paid, full-time social service experience and a bachelor's degree in social services/health sciences.
  • Social service experience and a Master's degree.
  • AIRS credentialing is expected within the first two years of employment.
  • Possession of a valid driver's license and reliable vehicle for work-related travel.

Responsibilities

  • Provides information by phone, email, mail, and in-person to the public and partners on programs and resources for older adults, disabilities, and caregivers.
  • Conducts eligibility screening and follows up on referrals as needed.
  • Offers person-centered options counseling resulting in linkage to services.
  • Assists individuals to complete applications for Medicaid Long-Term Care, SNAP, and Medicare Part D.
  • Promotes access to programs for those with greatest economic and social need.
  • Uses databases to document applications, eligibility, and authorization.
  • Records information and produces data reports.
  • Confers with community partners to coordinate care for clients.
  • Delivers in-service presentations and performs outreach at community events.
  • Performs outreach to promote agency programs and services.
