Job summary

Qualifications

  • Bachelor's degree in social work or a related field; one year of relevant experience required; master's preferred.
  • Experience working with aging and disabled populations preferred.
  • Knowledge of long-term care programs (e.g., Family Care, IRIS, AFCSP, SSIE, MA) preferred.
  • Knowledge of Trauma-Informed Care Principles essential.

Responsibilities

  • Facilitate consumer access to services and monitor contracted providers.
  • Develop and carry out service plans; coordinate and monitor services.
  • Administer mandated screening to determine program eligibility.
  • Provide short-term in-home or community-based case management.
  • Complete annual protective placement reviews for long-term care enrollees.
  • Complete abuse/neglect reports per state statute and refer for screening.
