Job summary

Walworth County’s Aging & Disability Resource Center in Elkhorn, WI is seeking a qualified information and assistance professional to provide options counseling, enrollment/disenrollment counseling, and long-term care program eligibility determinations to older adults and adults with disabilities.

The role emphasizes advocacy, collaboration with providers, and adherence to state and federal guidelines, with a Monday–Friday schedule and a 1.00 FTE workload.

Qualifications

  • Bachelor's degree in social work or a related field.
  • At least one year of relevant prior experience required; master's degree preferred.
  • Experience working with aging and disabled populations preferred.
  • Knowledge of long-term care programs such as Family Care, IRIS, AFCSP, SSIE, and MA.
  • Knowledge of Trauma Informed Care Principles is essential.

Responsibilities

  • Provide information and assistance, options counseling, and enrollment/disenrollment counseling.
  • Determine long-term care program eligibility and advocate for consumers.
  • Administer mandated functional and financial screens within state/federal guidelines.
  • Provide short-term case management including face-to-face contact in home or community.
  • Complete annual protective placement reviews for eligible individuals.
  • Advocate for consumers and educate their support systems to reach personal goals.
  • Collaborate with providers, other programs, and Health and Human Services staff.
