Job summary

Snohomish County Human Services is hiring Case Manager-Aging & Disability Services in Everett, WA. The role coordinates long term services for elderly and disabled individuals to promote independent living and prevent institutionalization.

You will develop service plans, assess needs, and connect clients with in-home support, reporting outcomes regularly. Qualifications include a Master's or Bachelor's in behavioral/health sciences and relevant social service experience.

Qualifications

• Master's degree in behavioral or health sciences or Bachelor's with 2+ years experience; or Bachelor's with 4 years experience.
• Background in social services and ability to pass tests.

Responsibilities

• Conduct comprehensive assessments to determine client assets, deficits and needs.
• Develop and implement service plans with tasks for case manager, client, and support network.
• Review service plans for changes and coordinate with community resources.
