Job summary

State of Washington Aging and Adult Services of Coastal Washington (AASCW) is looking for a part-time Case Manager based in Long Beach, WA. The role focuses on assessment, home-based care management, and service planning to enhance independence for frail adults.

You will work with a multi-agency team to deliver a broad range of services, maintain funding source compliance, and advocate for clients under the Older Americans Act framework.

Qualifications

• Master's Degree in behavioral or health sciences with 1 year paid social service experience, or BA in similar fields with 2 years paid social service experience; or BA with 4 years paid social service experience.
• Requires current WDL and insured vehicle.
• Schedule: 30 hrs./wk. Non-Exempt.
• Rate: $33.87/hr - $44.44/hr.

Responsibilities

• Provide assessment, reassessment and in-home care management services to eligible adults.
• Collaborate with client/decision-maker to create a service plan to maximize independence and quality of life.
• Coordinate, update and implement the client service plan within funding source requirements and agency benchmarks.
• Collaborate as part of a multi-agency service delivery team to provide needed services including advocacy.
