Responsibilities

• Provides case management services to seniors and adults with disabilities; conduct intake and comprehensive home assessments.
• Develops and implements care plans; coordinates services and follows up on progress.
• Maintains complete client files and data collection forms and reports.
• Reports incidents to appropriate bodies including Adult Protective Services.
• Conducts outreach and community education about elder abuse in the Chinese community.
• Participates in team meetings, training, and quality assurance activities.
• Performs other duties as assigned.
