CAPECO in Pendleton, OR is hiring a Case Manager to provide comprehensive care management for older adults and people with disabilities. You will assess needs, coordinate services, and help clients access Medicaid programs, home-based care, and community resources.

This role emphasizes client-driven plans and strong collaboration with partners. Regular full-time, non-exempt, primarily in-office with potential hybrid/remote options after training.

Qualifications

• Associate degree in related field or two years of equivalent experience.
• Experience with older adults or people with disabilities preferred.
• Ability to interpret instructions and maintain caseload records.

Responsibilities

• Conduct home visits to assess ADLs/IADLs and safety needs.
• Provide person-centered counseling and develop action plans.
• Coordinate in-home services and monitor client progress.
• Maintain client data in state databases and internal spreadsheets.
• Collaborate with agencies to coordinate services and referrals.
