Job summary

The State of Nebraska is seeking a Disability and Aging Service Coordinator in Lincoln to support individuals eligible for the HCBS waiver. You will manage service delivery, assess needs, and coordinate plans of services and supports under the Division of Developmental Disabilities.

You will develop individualized plans, arrange referrals, and monitor ongoing services while ensuring safety and well-being. A bachelor’s degree in a related field and valid driver’s license are required or

Qualifications

• Bachelor’s degree in education, psychology, social work, sociology, human services, or related field with experience in developmental disabilities or related services.
• Experience with person-centered planning, assessments, habilitation, and program implementation is preferred.
• Experience using Word, Excel, Outlook, Nfocus and Therap is a plus.

Responsibilities

• Coordinate and oversee the delivery of services to eligible individuals.
• Conduct assessments and determine eligibility for waivers.
• Develop and implement plans of services and supports.
• Provide referrals and monitor service provision.
• Ensure health, welfare, and safety of those served.
