Job summary

The Commonwealth of Pennsylvania, through the Northumberland County Area Agency on Aging, seeks an Aging Care Manager 2 to help older adults overcome challenges and stay safe in their communities. You will assess needs, coordinate services, and guide families, linking them to programs that simplify daily life and support well-being.

This full-time role follows a schedule of 8:30 AM to 4:30 PM, Monday through Friday, with no telework.

Qualifications

Six months as Aging Care Manager 1 or a County Caseworker 1, or equivalent.
Successful completion of the County Social Casework Intern program, or six months of professional human services or nursing experience.
Bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field.
Equivalent combination of experience and training.

Responsibilities

Needs Assessment: Evaluate social contacts, daily living activities, cognitive functioning, physical health, and available resources.
Service Coordination: Connect older adults with agencies, hospitals, clinics, courts, and community organizations that support their needs.
Comprehensive Reviews: Conduct detailed assessments of medical, social, psychological, and housing conditions to support effective care planning.
Interagency Collaboration: Work with professional staff and community partners to discuss diagnoses and plans of treatment.
Benefits Guidance: Assist older adults in managing income, accessing Social Security and Medicare, and understanding legislation that affects benefits.
Case Documentation: Prepare social studies, case histories, reports, and records.
